Ad relevance is part of the quality score Google gives each keyword in your campaign, and a low score means Google thinks your ad does not match what the person is searching for. This usually happens when the keywords in your ad group are too broad or too different from the actual words in the ad. For example, if someone searches for running shoes and your ad talks about gym equipment in general, that mismatch brings the score down.
