Don't do this. Extrapolate reasons for leaving from user behaviour patterns, don't interrupt their behaviour. Guaranteed never to return to your site if you do - and the ones who fill in your form will be guaranteed to be completely unrepresentative at best, and deliberately misinforming you at worst. I consider exit forms to be a UX antipattern. They will (rarely) be the only way you can get the information you're after. But first try to get that information in any other way.

Honestly, I agree that using a contract form as an exit pop-up can be tricky. Most people just want to leave a page and don't like being stopped by a pop-up, especially one asking them to sign something. Even if someone fills it out, the data you get might not be accurate because they're just rushing to close it. It seems safer to collect this info during normal interaction rather than interrupting them at the last moment.
