Ask Why do clients ask for more changes on work they already reviewed and approved?

One of the more frustrating things in freelancing is when a client reviews your work, says it is good, and then comes back later asking for changes that go beyond what was originally discussed. The approval already happened, so it feels like the project should be complete. But now there are new requests that were never part of the agreement. Some freelancers think clients do not fully review the work the first time. Others think they change their mind after showing it to someone else. What do you think is really behind this pattern, and how do you usually deal with it when it comes up?
