It's not 100% perfect—if someone has images blocked or uses a text-only client, it won't be counted—but it gives a good idea of engagement.
 
When someone opens the email, their device loads that image from a server, and boom. Most email tools like Mailchimp or Constant Contact do all this stuff automatically, so you don't really have to think about the tech side. Then they just take the number of opens and divide it by how many emails actually got delivered to get the open rate percentage. It's not 100% accurate though, since some people block images or have privacy settings that stop tracking, so some opens don't get counted.
